About the role:
We are recruiting to our GP project team in Mid Essex. Due to an increase in resources we are recruiting two 21 hours Dementia Support Workers to enable us to support more people affected by dementia.
The project offers support to GP practices and their patients, both before and after a diagnosis of dementia. The role will include running clinics at GP practices which will enable people affected by dementia to receive support and information, including local support groups, Lasting Power of Attorney, and practical advice on living well with dementia.
The successful candidate will also ensure that the information and resources about dementia is the surgeries are up to date and providing any new information needed.
A key part of this role is building and developing relationships with practices, running Dementia Friends and Awareness sessions for staff, enabling and supporting the practices to become recognised as a Dementia Friendly practice by guiding them through the accreditation tools. As part of this role we will support the successful candidate to become a Dementia Friends Champion.
You will need to be flexible in your work to ensure that we can provide support across all the GP surgeries that operate in the area.t is essential that applicants can travel independently within the Mid Essex area.
We are looking for an individual who possesses NVQ level 3 (or equivalent). Your ability to assess client needs along with the understanding of the needs for client confidentiality is essential. You will have excellent non-judgmental communication skills as well as great organisational and time management skills so that you can manage your workload effectively
You will be expected to work across the are so it is essential that you can travel independently.
