The role is to support people affected by dementia in the Hambleton and Richmondshire area.
As a Dementia Support Worker you will have the rewarding opportunity to provide support and guidance to people with dementia and their Carers; helping to maintain their independence; improving their sense of well-being; and putting them more in control of their lives. The service is person-centred and based upon personal circumstances and support needs.
Support Worker Responsibilities:
You will offer support and guidance to your clients in a variety of ways; face to face home visits; at service offices or other locations; by phone; letter or email. You will also assist people living with dementia to access other services, providing signposting and referrals, as well as facilitating support groups and other group activities.
You will maintain accurate, up to date client support plans and client contact records and you will seek and record client feedback to understand whether needs are being met, and to plan any necessary adjustments. You will work as part of the information and support team, providing cover for colleagues at agreed times or in unforeseen circumstances, as appropriate.
It is imperative to comply with data protection regulations, ensuring that information on clients remains confidential and to work in a manner that facilitates inclusion, particularly of people with dementia.
Support Worker Requirements:
We are looking for individuals educated to NVQ level 3 or equivalent (or demonstrable experience) to join our team in Tees Valley Hambleton & Richmondshire. The role is home based but there will be a requirement for visits to the local office base in Richmond North Yorkshire. The role reports directly to the Services Manager and will require you to have excellent non-judgmental communication.
