Support Worker

As a Dementia Support Worker you will assist people with dementia and their carers in identifying their needs, providing individuals with support and help to maintain their independence, improving their sense of well-being, and putting them in more control of their lives.

Support Worker Responsibilities:

Our client's Dementia Support service provides a highly responsive and individualised information, signposting and referral service to people with a diagnosis of dementia and those that care for them.

It will be important to ensure any helpful information is received and understood by the service user as well as helping them to access the appropriate services and contact the right people for the help they need. You will develop and contribute to a database of local information, ensuring continual improvement of the service and growth in organisational knowledge.

As a community Dementia Support Worker you will attend community events offering immediate support, or signpost for more specialised support. The post will offer dedicated support to community hubs starting with a particular focus on Stoke on Trent. By supporting community hubs we will increase our client's local presence and reach more people affected by dementia. Our client's community Dementia Support Worker would look to facilitate dementia friendly opportunities and reduce loneliness by capacitating more organisations to be able to support people with dementia. The effectiveness of the role will be defined by the number of events and people reached and/or supported. The role will also deliver Singing for the Brain, and support delivery of Carers Information and Support programme (Crisp) and Living Well with Dementia programme for people living with dementia.

This role will require you to build relationships with a range of local contacts. You will network with health and care professionals, marketing the Dementia Connect service to help people with a diagnosis to get referred to the service as early as possible. As well as developing networks with partner organisations who also work in the field of dementia to promote the aim of empowering individuals living with dementia to make informed choices.

Support Worker Requirements:

Our client is looking for an individual who possess NVQ level 3 (or equivalent) with an understanding of dementia and the needs of those living with dementia and their carers. Your ability to assess client needs along with the understanding of the need for client confidentially is essential to providing a valuable and worthwhile service. You should have experience of awareness raising within communities and networking abilities. To be successful in this role you will have excellent non-judgmental communication skills as well as strong organisational and time management skills in order to manage your caseload of clients effectively.
